The departments' Community Paramedic program was introduced in 2017 and works with a number of community stakeholders, including Bluewater Health, primary care, home and community care, to provide case management, coordinated care planning and other proactive measures in the community versus in the traditional 'bricks and mortar' health settings. We work with community partners to ensure patients receive the right care at the right time.
The paramedics assigned to this program have received additional training through a partnership with Fanshawe College and are supported by a number of local health providers.
The goal of the program is to reduce unnecessary 911 calls, emergency room visits and hospital admissions by helping at-risk residents stay healthy and happy in their home.
